What is a booking form?

A booking form is a form used to collect information about an occasion that you have been invited to. 


When should a booking form be used?

A booking form should be used when you receive an invitation to speak, perform, provide a service etc for an occasion and you need to gather details to determine whether you will accept or decline the invitation.


Who should complete the booking form?

A booking form should be sent to the person/group/organization you are inviting you. When they complete the booking form, they should send it back to you for review.


How does the booking form get completed?

When you send the person/group/organization that has invited you, they should complete the form by providing as much information as possible about their occasion for you to review.
